If you missed this round of payments or were unable to submit your forms in time — don’t worry. Eligible members have up to 5 years to receive their PCD through the Peter Chapman Community Trust.

B. Copy of your IDs

C. Signed BMO Waiver Form (Download Form Below)

 

Eligibility Criteria:

1. You must be a registered member of Peter Chapman Band

2. You must have a valid status registration number

3. You must be on the Peter Chapman Band list as of January 11, 2025

If your banking information was incorrect and the deposit was returned, Melinda will contact you directly to make sure it gets reissued.
